fix(qt): the replacement modal spells out each candidate's quality

The tier badge alone reads as a mark, and its hover tooltip never shows
on these rows because the row's MouseArea takes the hover. The exact
tier ("24-bit / 96 kHz") now sits beside the badge — it is the decision
aid the list exists for.
