Subject: [PATCH] Change default settings for Rate Limiting and Fuzzy Match
 Threshold

Changed default values:
- Rate Limiting: "medium" → "none" (disabled by default)
- Fuzzy Match Threshold: 85 → 65 (more permissive matching)

Updated all fallback values in settings.get() calls throughout the codebase
to ensure consistency with the new defaults.
